STATEMENT OF WORK





FOR CARD ACCESS MAINTENANCE SUPPORT AFLCMC, GUNTER ANNEX, MAFB






  1. DESCRIPTION OF SERVICES: The work covered by this specification consist of furnishing all plant, labor, material (parts based on reimbursable rate), tools, and equipment to perform all operations in connection with monitoring, inspection, maintenance, and call-back repair service of government owned card access system at Gunter Annex, Maxwell Air Force Base, in Montgomery, AL as described in these specifications. Prior to submission of an offer on this contract, it is suggested the contractor visit the site for the purpose of determining exact requirements.






  1. EQUIPMENT LISTING: A listing of equipment to be inspected, maintained, repaired, and serviced under this contract is contained in Paragraph 7. Equipment not listed, but which is required to make the system function properly, shall also be maintained. This additional equipment should be verified during a site visit prior to offer submission; e.g. associated wiring and sensors, etc.





EQUIPMENT AND DEVICES: All equipment and devices utilized within this system shall be approved by Underwriters Laboratories (UL), National Fire Protection Agency (NFPA), , AFI 16-1404, Air Force Information Security Program, and when specified, shall be as listed as approved equipment in the latest UP 1706 and/or DCID 1/21 document approved by the Director of Central Intelligence.






  1. DESCRIPTION OF WORK: Work shall include the following:








    1. ANNUAL MAINTENANCE AND INSPECTION: At least once yearly, the contractor shall inspect all installed system components and perform all preventive and corrective maintenance and inspections, the contractor shall provide the Quality Assurance Inspector (QAI) with a neatly typed report of system conditions and work performed (see paragraph 7 for component listing). Report shall contain the following information:












      1. Detailed evaluation of system condition

      2. Summary of work performed during annual maintenance and inspection.

      3. Recommendations to improve the system reliability.









3.2 SCHEDULING OF WORK: Prior to performing scheduled inspections and maintenance, the contractor shall notify and coordinate in writing and verbally at least five (5) working days in advance with:

The government must pre-approve dates prior to going on-site to perform any work during normal duty hours (0730-1600 Monday thru Friday) and the contractor shall sign in with Security Office, Bldg. 892, Room 130 on Gunter Annex. Annual preventive maintenance shall be scheduled between June and August.






  1. CALLBACK SERVICE: Callback service is defined as additional visits and services requested at any time other than regularly schedule visits.








    1. SYSTEMS: In the event of system failure, the contractor shall respond to begin site repairs no later than the next-business day (from determination of need to dispatch) after receiving the repair call from personnel listed below in Paragraph 4.3. The Contracting Officer (CO), Contracting Officer Representative (COR) or QAI must approve all repairs exceeding a total of four hours from the arrival on sight.










    1. CALLBACKS: Callback reports/service tickets shall include the trouble condition and its resolution for the buildings, problems, and repairs made on callback service. These reports/service tickets shall be turned into the QAI at Security Office, AFLCMC GB, Bldg. 892, Room 130, prior to leaving the base after completion of repairs.












    1. AUTHORIZED PERSONEL: Upon award of this contract, the contractor will receive from the contracting officer a list of persons holding positions who will be authorized to make callback service calls. The contractor shall notify one of the authorized government representatives by telephone prior to beginning work and upon completion of the service call.












      1. All requests for service to be called in to Advantor 24/7 Tech Support line, as the dispatching center for Advantor.










  1. CONTRACTOR QUALIFICATIONS: The contractor/personnel performing the work under this contract shall maintain a current workforce highly experienced in the inspection, maintenance, and repair of equipment listed in Paragraph 7 and plus those items not listed, but viewed during the site visit prior to submission of an offer to this solicitation. The contractor shall submit a listing of personnel to perform the work with qualifications and certification to the CO with the bid proposal.





The contractor is not required to maintain a current workforce certified on the Advantor System but shall maintain one business day access to personnel that hold Advantor certifications, available to make onsite repairs if required, since the software that operates the card access system requires certification from Advantor.






  1. CONTRACTOR RESPONSIBILITIES: The contractor shall maintain a sufficient stock and have a means to expedite and all parts necessary for the equipment listed in Paragraph 7, in order to meet the requirements stated in Paragraph 3 and 4. The contractor shall provide all necessary parts to accomplish maintenance of existing system components and shall proceed with repairs, ONLY AFTER RECEIVING APPROVAL TO DO SO from persons provided on the list from the Contracting Officer. The service representative shall then prepare a service ticket itemizing all cost, including parts and labor used. The contractor shall be reimbursed for labor expended for Callback service at the contract hourly labor. The contractor shall be reimbursed for all parts including direct cost and transportation and shall provide proof of cost per Government Discounted Rates. The contractor shall insure such parts are furnished at a fair and reasonable price, which is no higher than that charged to any other Agencies. Upon request, the contractor shall provide copies from three suppliers validating coast of a specific item showing manufactures suggested retail price. Immediately after maintenance or repair is performed the contractor's service representative shall turn over all removed defective parts and a copy of the service ticket to approving personnel provided on the list by the Contracting Officer. Travel time shall be billed separate from regular hours. The contractor shall provide a phone number for a technician/systems administrator that could be reached during normal duty hours for answers to resolve technical problems within the existing system.








    1. The contractor shall be responsible for obtaining any manuals necessary to provide service in this contract.










    1. The contractor shall use all practical means to ensure the systems are down only for minimal time. Employee vacation, sickness, or lack of availability of parts is not accepted as an excuse for having the system down.








  1. GOVERNMENT OWNED EQUIPMENT, GUTNER ANNEX: The following is a list of government owned card access system components and related equipment at Gunter Annex, Maxwell AFB, to be maintained under this contract. Equipment not listed, but which is required to make the system function properly, shall also be maintained. This additional equipment should be verified during the site visit prior to offer submission: e.g. associated wiring and sensors, etc. At Gunter Annex, the card access system was installed by Advantor Systems Corporation and operates on their software.

  1. HOURS OF OPERATION: Work shall be performed during the normal work week, 0730 to 1600, Monday thru Friday.






  1. PRINCIPLE PERIOD OF USE (PPU): The PPU for all components of the card access system shall be 24 hours a day, seven days a week.






  1. RECOGNIZED HOLIDAYS: The contractor shall not schedule inspections and maintenance on the following federal holidays.

  1. PERSONNEL: Contractor personnel shall present a neat appearance and be easily recognized while on base. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. The contractor shall be responsible for acquiring an appropriate number of badges to meet his/her needs at the contractor's expense. The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD 5500.7-R Joint Ethics Regulation (JER) dated 17 November 2011. The contractor is cautioned that off-duty military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployed. Military Reservist and National Guard members may be subject to recall to active duty. The abrupt absences of these personnel could adversely affect the contractor's ability to perform. Their absence at any time shall not constitute an excuse for nor-performance under this contract.








    1. CONDUCT OF EMPLOYEES: The contractor is fully responsible for the performance and conduct of his/her employees at all times while performing under this contract. The contractor shall not allow any employee to perform work under this contract while under the influence of alcohol, drugs, or any other incapacitating agent. Security Police and other law enforcement personnel on base have the right to deny entry to the base and remove from the base any contractor employee for misconduct that endangers the health and/or safety of people or property or for reason of security.







SECURITY REQUIREMENTS FOR UNCLASSIFIED SERVICES






  1. Contractor Notification Responsibilities: The contractor shall notify the AFLCMC Contracting Office before on-base performance of the service. The notification shall include:






  1. Name, address, and telephone number of company representatives.






  1. The contract number and contracting agency.






  1. The location(s) of service performance and future performance, if known.






  1. The date service performance begins.






  1. Any change to information previously provided under this paragraph.






  1. Installation Access Requirements: As prescribed by the AFFAR 5352.242-9000, Contractor access to Air Force installations, AFI 31-218, Motor Vehicle Traffic Supervision, and AFMAN 31-113, Installation Perimeter Access Control.





The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.





The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration and a valid vehicle insurance certificate to obtain a vehicle pass.



Government ID card holders may not use their credentials to sponsor an individual on to the base to perform work that’s been contracted to an official contractor.





During performance of the service, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials for any employee who no longer requires access to the work site.





All commercial vehicles will be directed to Gate 4 (Congressman Dickenson for Gunter) for processing through the Commercial Vehicle Inspection (CVI) Area, when operational. During non-duty hours and holidays, commercial vehicle inspections will be conducted at Gate 4 only. Inspection members will conduct thorough inspections of the interior and exterior of the vehicle for items prohibited from the installation such as explosive devices, weapons and ammunition, drugs, and open or closed alcohol containers.






  1. Entry Procedures for AFLCMC Gunter Facilities: Contractor employees require an AFLCMC BES Gunter Access Badge for unescorted entry into AFLCMC BES Gunter facilities. To obtain an AFLCMC Gunter Access Badge contractor personnel must be in JPAS or have a VAL on file with AFLCMC Gunter Security Office. Contractors requiring an access badge will bring a completed AF Form 2586, Unescorted Entry Authorization Certificate to the AFLCMC Gunter Security Office Room 130 Bldg. 892. The AF Form 2586 must include in Section III the task order number, period of performance, facility number and be signed by the Contracting Officer Representative (COR) associated with the assigned contract. Contractor personnel are required to wear their company’s identification badge and the AFLCMC Gunter badge AT ALL TIMES while in AFLCMC Gunter facilities. The access badges will be displayed on the outer garment between the shoulder and the waist. Every time a period of performance is executed this paperwork must be re- accomplished to reflect the new expiration date.





Failure to return all government issued identification upon termination of contract or service, termination of employees, or expiration of the identification may result in withholding of final payment.






  1. Reporting Requirements: The contractor shall comply with AFI 71-101, Volume-1, Criminal Investigations, and Volume-2, Protective Service Matters, requirements. Contractor personnel shall report to 42d Wing Information Protection Office, and AFLCMC Gunter Security Office, any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment and as required thereafter.






  1. Traffic Laws: The contractor and their employees shall comply with base traffic regulations set forth in AFI 31-218, Motor Vehicle Traffic Supervision and AFMAN 31-113, Installation Perimeter Access Control. Personnel in violation may be issued a Central Violations Bureau Form 1805 traffic ticket.






  1. Random Installation Entry/Exit Checks: Entry/exit vehicle checks are conducted by order of the 42 ABW Commander. These checks are conducted for the purpose of safeguarding the base and protecting government property by discovering and seizing stolen property, classified information and contraband. Refusal to submit to an installation (entry/exit) vehicle check may result in the loss of base driving privileges, revocation of base registration, or debarment action.










  1. Cellular Phone Operation Policy: The use of cellular phones while operating a motorized vehicle is prohibited on Maxwell-Gunter. Although discouraged, drivers are authorized to use devices, i.e., ear bud or ear boom, which allows their cellular phone to be operated hands-free. The device must not cover both ears. This policy applies to everyone driving on Maxwell AFB and Gunter Annex. Personnel in violation may be issued a Central Violations Bureau Form 1805 traffic ticket.






  1. Wireless Electronic Devices: The contractor will not establish their own Information Technology (IT) systems or networks (Local Area Networks [LAN], Wide Area Network [WAN], Cellular phone/USB Modem as WAN, Wi-Fi as WAN, etc.), or camera system without the direct permission of the Program Manager and governing communications and responsible information systems office (42 CS).






  1. Firearms and Ammunition: Transporting weapons or ammunition, concealed or otherwise, IS NOT permitted by any non-law enforcement personnel on Maxwell AFB/Gunter Annex at any time regardless of state issued concealed weapons/LEOSA permits. Violations may result in criminal prosecution under the applicable federal laws.






  1. Illegal Weapons. The below weapons are considered illegal, unless specifically authorized by competent authority, and are prohibited on Maxwell AFB/Gunter Annex. Violations may result in criminal prosecution under the applicable federal laws.




  1. Switchblade knives or knives with any type of automatic blade release.

  2. An incendiary/explosive weapon (e.g., grenades, flash bangs).

  3. Fireworks

  4. Homemade mortars, aka "tennis ball launchers" or similar devices.






  1. Anti-Terrorism (AT) Training Requirement. The objective of Level I - AT Awareness Training is to provide all personnel with requisite knowledge necessary to remain vigilant for possible terrorist actions.
